What should I bring to my first appointment?

Please bring your physician referral if you have one, your insurance card(s), MRI results if applicable, and any correspondence from your doctor. If you are a Workers’ Compensation patient, bring your claim number, name of the insurance company, and case manager’s name and contact information. If you have been involved in an auto accident, bring the name of the insurance company, claim number, adjuster name and contact information, and a copy of the police report. If you are involved in any other type of accident, the same information applies except the police report. If an attorney is involved in your case, please bring their name and contact information with you also.
